We are so glad you are coming!  If you would like to fill out your paperwork before the day of your appointment, you can fill it out either on-line or print them from home.

       

 

Medical Release Forms

To release medical records every doctor's office needs written permission to do so.  You can pull up a medical release form by clicking the GREEN BOX below .Once you print your copy, sign your release and fax it back to us so that we can have your release to allow us to fax or mail the information you need.

If you need a sports form, camp form, school form, or immunization records faxed or mailed, use this form:

 

 

If you are transferring your child's care TO our office at Pediatric Associates at Franklin and need to get a medical release for ALL your old records at another office to send to us, use this form:

 

 

If you are transferring your child's care FROM our office at Pediatric Associates at Franklin and need to get a medical release for your current records from US to send to a different medical office, use this form:
